What Is It?


NOTE: Make sure you have contacted PWC Employee Relations (Irina Shashkova or Patty Turville) about this type of request and have been directed to this form.

**Options selected on this form will only stay in effect until the account is DELETED by the automated system, once the Separation PAF and final termination paperwork has been completed by all required departments. **

It should also be noted that if the employee is ALSO a student, the only available options are Lock Banner Access and Forward Phone. All other options would disrupt student activities.

Exceptions in extreme circumstances.

Who or What is Eligible?


Only Supervisors with direct reports should use this form. This is to be used in urgent cases that involve the Employee Relations team. It is not for standard offboarding of employees.
