OIT User Account Exemption Form: Before and After Hours

Objective: Ensure timely and secure deletion of accounts after regular college hours.

You will likely be directed to the OIT User Account Exception Form by Employee Relations. You can fill out and submit this form, but Employee Relations must be involved. Contact them at 801-957-4427.

If you cannot reach Employee Relations before or after regular college hours, please contact the Help Desk at 801-957-5555. The Help Desk can assist you in contacting an Employee Relations employee. You may also submit a ticket from the support site.

The Help Desk cannot see the form that has been submitted, for security and privacy reasons. They will create a ticket and reach out to the required IT member(s) to help facilitate and expedite the request.

Please include the following:

  • Short Description: "Urgent: After-Hours Account Deletion Request"
  • Comments: Include all relevant details from the call, such as the caller's information, supervisor information, the user being locked out, whether it's a Mac or PC, and the time the form was submitted and Employee Relations employee.
  • Priority: Set Impact and Urgency to “High,” which will set the ticket priority to "Critical."

Regular college hours are M-F 8am – 4:30pm. The Help Desk is open M-Th 7am-8pm and F 7am-6pm. Saturday emails are checked on a limited schedule. *For Summer hours, please see the bottom of the Support Site (support.slcc.edu).
