Request Account for Non-SLCC Employee for Network Access

What Is It?

Salt Lake Community College (SLCC) provides system access accounts for non-SLCC employees, such as contractors, when their work requires interaction with SLCC systems or resources. These accounts are created to ensure secure and appropriate access while maintaining compliance with institutional policies. Contractors must be sponsored by an SLCC department and follow the established procedures for account request and approval, including verification of their role and duration of engagement. This process helps SLCC support external collaboration while safeguarding internal systems and data.

Who or What is Eligible?

This request is available for Contractors with an SLCC Sponsor that require specific access only granted by an SLCC account. If you do submit this form, you are agreeing to the following standards for SLCC network access:

By signing this document and accepting this SLCC network account, I understand the following:
1.    I understand that either I or the Hosting Organization is responsible for obtaining a system that is compatible with the SLCC wireless network.  The wireless card must be compatible with WPA-2 Enterprise wireless authentication.
2.    I am fully responsible for this network account (password, etc).
3.    I must have approval from my SLCC hosting organization before I will be permitted to access the SLCC network.
4.    While I am connected to the SLCC campus network, I am responsible for complying with the SLCC Acceptable Use Policy for Computer Usage (https://www.slcc.edu/policies/policies/finance_and_administration/2.3.010.aspx). Any violation will be handled by the College's legal representation.
5.    I am responsible for maintaining current virus protection and Operating System patches on any Non-SLCC wireless device I use to connect to the SLCC campus network.

Verification of adequate virus protection and current O/S patches on my laptop should be coordinated with Help Desk personnel located in TB 121 on the Redwood Campus (801-957-5555) before being issued network access/account.  Introduction of any virus to the SLCC network environment (whether intentional or unintentional) may result in a fine and/or suspension of network privileges.

6.    I will consult with OIT personnel for any needed technical support. I am responsible for any damage to this equipment.

Current virus protection and security patches not only protects SLCC assets from potential problems with your computer, but also protects you from any problems that may be in the College network.  SLCC personnel are responsible only for providing media and basic instructions for loading the network card drivers and virus protection software.   The user is personally responsible for installing such software or drivers and SLCC assumes no responsibility if the drivers or virus protection software do not function properly or if they cause any disruption to the normal operation of the user's laptop computer.
